Intake Appointment Checklist
This resource outlines Compass' structure for FSS intake appointments. This intake appointment is the first appointment in the coaching relationship where participants sign and complete the COP and ITSP.
Pre-Appointment
□ Review notes from any previous communication with client
□ Compile necessary forms and resources for the appointment
□ Review client’s credit report
□ Contact client with reminder of what information to bring to appointment
Welcome & Administration
□ Review appointment agenda with client
□ Review Contract of Participation (sign)
□ Review any other program paperwork (sign)
□ Collect all signed documents to copy later
Getting to Know the Client
Take notes during your conversation and:
□ Ask about household & dependents
□ Ask about education
□ Ask about employment
□ Ask about goals
□ Ask about finances
Client Goals
□ Complete ITSP with major program goals
□ Discuss and recommend any additional goals and input into ITSP
□ Print & sign ITSP
Budget
□ Collect pay stubs to copy later
□ Input paystub information into data system
□ Ask about other sources of income (Child support, etc.)
□ Input assets info
□ Input tax info & set aside to copy later
□ Take note of public benefits the client receives
□ Review and complete monthly budget
Credit Report
□ Review all details of credit report
□ Discuss goals and input into ITSP
Discuss Other Resources & Wrap Up
□ Ask client if there’s anything else they would like to go over
□ Make copies of all necessary documents
□ Print budget summary for client
□ Final review of goals and print for client
□ Schedule next appointment
Post-Appointment
□ Upload or file the pay stubs, tax info, and other relevant information in data system
□ Complete post-appointment narrative
□ File and share COP and ITSP with appropriate team members
□ Send follow-up email to client with any next steps or additional resources
